Q: In Atlas America, After deductible, pays 100% (URC out of network) to policy maximum, what do you mean by "URC out of network"?

"URC" are the Usual, Reasonable and Customary charges in the area from which services are provided. If an individual goes outside of the PPO network and received treatment there are no network negotiate rates that are offered. Going out of network the provider is going to charge their standard rates for the services rendered. When this is done the plan will only cover what is considered to be a standard rate in the area that service were render in. So for example, if a charge of $1,000 was issued for an out of network expense and the URC in the area is $600 the plan will only pay $600 for that claim.
