Q:My mother is 73 years old and currently in US on visiting visa. She has BP and Sugar as pre existing deseases. My brother is already citizen of USA and not added her as dependent in his current family policy. We are planning to apply Green card processing for her so that she can stay longterm in USA with my brother. Q:My mother who is visiting us in USA fell down on the sidewalk and got an abrasion on chin. So, we went to the urgent care to get a tetanus shot for her. The urgent care charged 70 for the shot instead of 30 copay as mentioned on our First Health insurance. Please let me know which form to use for filing the claim.
